There are times in our lives when we think about the consequences of

becoming seriously ill or disabled.

Advance care planning’ (ACP) is the process of enabling a person to

think about the impact of future illness or disability and express wishes

about his or her future care in consultation with their care providers,

family members and other important people in their lives
